Lineup packages should be smaller and split by RID not TFM

Today the lineups contain the universe. Instead they should just contain the following:

1. Any package that added support for any TFM applicable to this RID in a newer version.
2. Packages in NETStandard.
3. Packages which require a RID-specific implementation.
   Issue - corefx#1 should really only be the set of packages that were previously in the PCL profile since those didn't include support for all TFMs.

Some more thinking on this in lieu of the fat-app plan & generations.

As part of the work to enable portable applications, the only packages that will be split are the following:

1. packages in NETStandard.Platform
2. native shims

As part of generations there is no longer any side-effects of upgrading dependencies. As such we shouldn't need to automatically lift dependencies to the latest version. Instead folks can just upgrade to latest to ensure they support the most platforms.

These two factors remove a lot of the TFM/RID specific qualities of the lineup. As such we should do away with split lineups and have one common one that is used for all packages. This lineup should only have entries for the packages mentioned above.

The lifting behavior should be removed from the default lineup. Instead we can put this in a seperate package that folks can use if they desire. (We have taken a dependency on this behavior for testing different baselines)
