for issue #14 -

I have added an additional question that deals with the inclusion of regional minority groups. The question has an option of answering 'No' or 'Yes' while specifying the name of the minority (and location?).

Thanks! This is a great first start. I think in the "do you identify as a minority" section, it makes sense to just have a yes/no/i'm not sure/i'd rather not say option. People can identify as part of several minority groups at once, and until we collectively get a better handle on how we' want to ask that, having a yes, self identify here feels confusing.

Does that make sense?
