dylan-tool has now subsumed all make-dylan-app functionality, so remove make-dylan-app and update the getting-started-cli documentation to reference dylan-tool instead, and update a few procedures related to registries and LID files that are different for dylan-tool.

I'm being conservative about removing the content about maintaining Git submodules because
dylan-tool
still doesn't have a reasonable way to handle non-public dependencies. i.e., some kind of local package catalog that augments the public catalog.
