Incorporates two changes:

  1. The suggested change here: Simplify envelope secret derivation cfrg/draft-irtf-cfrg-opaque#126
  2. Ensures that the mac generated by Km3 in 3DH covers the Km2 mac.

This requires a change to the test vectors.
