Updating examples to match the new fastify plugin architecture #91

Changes all example imports to match the new class names (Card -> CDSCard, etc.)
Replaces the Http, start, and Config modules with fastify's modules (see Migrate CDS Hooks + Rest modules to fastify-plugin architecture #86)
Updates all to the most recent version of Sero (^0.0.19)
