feat(action): emit publish request issue URL as annotation#708

Capture the URL returned by gh issue create and emit it as a ::notice:: annotation visible in the GitHub Actions workflow summary. Also exposes the URL as an action output (issue_url) for downstream steps. Handles both new issue creation and existing issue detection.
