Reduce staged preview item headers from h3 to h4

Staged mode previews in GitHub Actions summaries now use h4 (####) headers for individual items instead of h3 (###), improving visual hierarchy under the h2 main preview title.

Changes:

• Updated renderItem functions in:
  • create_issue.cjs - Issue previews
  • create_pr_review_comment.cjs - Review comment previews
  • update_release.cjs - Release update previews
  • update_runner.cjs - Generic update previews (issues, PRs, discussions)
• Updated test expectations in corresponding test files
