Historic Milestones are still useful for orientation and are good visible:

What about reusing obelisk or guidepost icon in different colour?

@Tomasz-W, that's what I was thinking. Reusing the obelisk icon sounds good. It would have to be a little shorter and wider though. I think milestones are closer to the ground and more round then obelisks tend to be.

Please not the guidepost icon.

Anything, that looks like a massive (and often high) block of stone.

Still shorter and fatter then an obelisk though.
oblisk 1
obelisk 2
Their different definitionally also.

Milestone "a stone set up beside a road to mark the distance in miles to a particular place."

Versus

Obelisk "a tall, four-sided, narrow tapering monument which ends in a pyramid-like shape or pyramidion at the top."

milestone without the base

other similar historic icons have a base. is there a special meaning or just a case of design/style?

Historic icons with base:

memorial:

statue:

shrine:

obelisk:

bust:

There are lots of milestones out there

there are only 3900 entries in the database.

Do you mean historic=boundary_stone (28500 entries)?

An interesting historic-value either, but much smaller.

Should we consider the icon-size to a future historic=boundary_stone ?

Personally like the icon without a base, coming out of the ground is what they do around where I live.

The icon suggested by Tomasz-W looks fine.
