Breaking changes
Cursive::{ncurses, pancurses, termion}
now return
io::Result<Self>
instead of panicking. Cursive::default()
still unwraps.

Improvements
Updated termion backend to use direct /dev/tty access for improved performance.
Enabled raw mode for ncurses and pancurses. Among other improvements, this
