Updated dependencies

Added .gitignore and package-lock.json.

Removed unused dependencies and requires using them.
Updated remaining dependencies to modern versions to remove several security vulnerabilities.

All tests pass, though it can't push the changes to coveralls.io due to it being a different repository.

As mentioned earlier, the travis-ci build fails due to the following issue, not due to a lack of tests passing:
