This are my changes to the citeproc branch as discussed on the mailing list.
Additionally I added some basic tests for citation support using Andreas files, so I have some minimal automated testing of my refactors.

Closed it as this approach is not general enough.

This is enabled by default in pandoc and GitHub markdown but not the
other flavors.

This requirse a space between the opening #'s and the header
text in ATX headers (as CommonMark does but many other implementations
do not).  This is desirable to avoid falsely capturing things ilke
