pip uninstall hide_code does not uninstall the extension from jupyter

Looks like pip doesn't support uninstall hooks so there will be some hide_code remnants after uninstalling. in the 0.4 release, nbextensions is supported. I'll handle removing the files pip doesn't during the upgrade process.
