Upgrade to FHIR STU3 v1.6

This release upgrades the FHIR server dependency to STU3 v1.6. It also switches the dependency manager from Godep to Glide.

Finally, the Ruby dependency has been eliminated from the project. Previously, this project used the health-data-standards library to import eCQM bundles. That functionality has now been incorporated into the JavaScript measure calculation engine.
