Add Windows support with CI

This project needs to have Windows CI to ensure all features work intended on Windows.

At least #117 may be a problem.

AppVeyor CI is the easiest to set up, with https://packaging.python.org/guides/supporting-windows-using-appveyor/ being a good guide
