Title: Add Form Submission Handling and Export Functionality (CSV/Excel)
Description: This PR introduces the following functionalities to the user registration form:
Form Submission Handling: Added a JavaScript event listener to handle form submissions. The form data is captured and logged to the console.
Data Export: Users now have the option to export the form responses in two formats:
CSV: Form data can be exported as a CSV file.
Excel: Form data can be exported as an Excel file (.xlsx).
User Prompt: After submitting the form, users are prompted to choose between CSV and Excel export formats, ensuring a flexible experience.
This feature enhances the form’s functionality by allowing data export for easy record-keeping and analysis.
