If the mbf_action_recovery_behaviors parameter is unset, then when the local planner aborts, abstract_navigation_server will SIGSEGV. This is reproducible every time.

Ugh. Just today, much new code has been pulled into master, so I am going to close this. If it reproduces on the current master, I'll report this again.

- In RefinePlan service callback if the global planner couldn't find
 a global plan and also the `cancel` method was not implemented
 aggressively stop the planner by interrupting its thread.
