in the config, the organisation field can be an org slug, or user/{user-slug}

script is called thusly:

$ API_TOKEN=[redacted] python codelists/scripts/bulk_import_codelists.py [/path/to/csv.csv] codelists/scripts/ad_hoc_old_style_snomed.json --live-run
