iD adding uncommon healthcare:speciality values

Describe the bug
iD is adding values like ear;nose_&_throat or dental;oral_&_maxillofacial_surgery which are used very rarely and probably result of some bug.

To Reproduce
Steps to reproduce the behavior:

1. find or add a doctor
2. select "Dental, Oral & Maxillofacial Surgery" or "Ear, Nose & Throat" in the Speciality field
3. see 2 specialities added in Speciality field instead of one
4. see dental;oral_&_maxillofacial_surgery or ear;nose_&_throat is tagged

Expected behavior
Use the more common and documented tags dental_oral_maxillo_facial_surgery or otolaryngology
