Delete the checked in file permanently from repo and commits.

Hello

I mistakenly added email username and passwords of my companies servers in one of the file.
I deleted the file and committed the changes. But I can still see the file content in commits.
Please let me know how to delete such files permanently from every where.
I tried couple of the steps from intranet but could not succeed.
I tried:

1. git filter-branch --index-filter 'git rm -rf --cached --ignore-unmatch monitoring/server/email_alert.py' HEAD
2. git filter-branch --tree-filter 'rm -f monitoring/server/email_alert.py' HEAD
3. git rebase -p --onto --rebase-merges SHA^ 8ac83144f1ee2061f170b6defacdec46b566422f

But nothing worked.

Im honestly surprised that there is no great way to remediate this from github today; in the instances of secret leaks, it would be good to either mask or delete commits.
