Create snapshot modal

Since there could be a lot of disks in a project, I'm not sure if we'll want to go for the disk select version, at least not until we can make it a combobox that lets you search by name. It might be easier to start out with this modal only accessible from disk detail and/or the disk actions menu on the disks table (whether on the full disk list or the list of disks for an instance). In practice I feel like the user is going to be starting from knowing the name of the instance they want to snapshot, not the name of the disk.

Interesting, could we go for a select / combobox for the instance which reveals a second select for disks that belong to that instance if it has multiple disks?
