kexec doesn't quite work. #27

There are some issues with booting kernels using kexec (it doesn't work). It would be great if someone could look into it. I know there's lots of interest in being able to select kernels and rootfs without modifying files and kexec is probably the simplest, so it's definitely worth fixing, I think.

The power management code is probably blocking when it tries to power up the devices a second time.

Does it boot the first time?

If by first time you mean just a standard boot, then yeah everything works just fine.

Then, without Simon's patch, when I kexec the kernel again, it says something to the effect of "Starting new kernel... bye!" and nothing happens. I haven't tried looking at the serial port output yet though. Well, I know it starts to boot, but it doesn't get very far along... not far enough to put anything on the screen.

The patch works, thanks. However, the resolution is reset to something low, so the hdmi_mode option is ignored. Also, there are some MMC errors, but it still boots everything fine.

I'll post more info (the exact errors) once I get the serial port working.

Check the parameters you're passing to bcm2708_fb on the kernel command line.

After playing around with it a bit more, yeah, it definitely works great. Haven't run into any issues yet.

I've run into some trouble. Video core doesn't quite work after a kexec boot.

When launching OpenELEC and hello_triangle.bin I get the following errors:
vchiq_get_state: g_state.remote->initialised != 1 (0)
vcos: [1679]: vchiq has no connection to VideoCore

kexec support is still labeled as experimental on the ARM architecture in the official release, this should really be reported (and followed for updates) there. As far as I know this is one of the big areas of focus for kernel development. Although, on the Pi it really isn't that useful from what I can tell, I clocked rebooting both through kexec and normally, and get only sub-second differences in timing.

system - and doesn't add any value. Drop this message.

Reproducing is easy, I can hit it on a KVM system with a very basic
config (x86_64 make defconfig + enable the drivers needed). To hit it,
just install dump (on debian/ubuntu, not sure what the package might be
called on Fedora), and:

dump -o -f /tmp/foo /

You'll see the warning in dmesg once it forks off the I/O process and
starts dumping filesystem contents.

I bisected it down to the following commit:

    This will be used to associate bio's with a given task.  This patch
    doesn't introduce any visible behavior change.

It seems like the init of ioc->nr_tasks was removed in that patch,
so it starts out at 0 instead of 1.

Tejun, is the right thing here to add back the init, or should something else
be done?

The below patch removes the warning, but I haven't done any more extensive
testing on it.

Hi, I'm wondering if something has broken in kexec since these messages here. I'm running the latest kernel and firmware (as of 3rd Dec 2012) and I can't get get kexec to work at all. I posted a detailed summary of the problem over on the RPi forums (here http://www.raspberrypi.org/phpBB3/viewtopic.php?f=28&t=24670&sid=bea05ce6eda0f6c50fb2c99ccbb2ab33). The quick summary of that is that kexec just gives a message about starting the new kernel, but then there's no output from the new kernel. I put in some printks to see which bits were succeeding, and it seems that we never return from machine_kexec() and that the problem lies somewhere in cpu_reset(), which is the last line executed in there.

Does kdump work? On some arches, kdump doesn't use exactly the same code as kexec, and if it works in this case it will make it much easier to figure out what is wrong with kexec.

Using a kernel built with earlyprintk it looks like the scheduler isn't switching to the kthreadd task for some reason. In the trace below the dd82a040 task is kthreadd and __schedule() is about to context_switch() to it. The "f" printk is at the bottom of finish_task_switch(), but a printk right at the very bottom of context_switch() doesn't appear (after return from finish_task_switch). Or perhaps that's normal - I'm not too familiar with scheduler code, and printk side-effects.

Any ideas where to look further as a culprit? My first guess would be something memory or cache related? Disabling I- and D- caches didn't help.

Kexec seems to work fine booting rpi-3.9.y , booting either from that or from the 3.6 kernel. That'll teach me for not trying the newest version first!

Nothing's changed on our side that I can think of that would fix this. Perhaps there was some upstream fix.

Still having trouble with a kexec't kernel:

The problem is that the slave is first linked and slave_cnt is
incremented afterwards leading to a div by zero in the modes that use it
as a modulus. What happens is that in bond_start_xmit()
bond_has_slaves() is used to evaluate further transmission and it becomes
true after the slave is linked in, but when slave_cnt is used in the xmit
path it is still 0, so fetch it once and transmit based on that. Since
it is used only in round-robin and XOR modes, the fix is only for them.
Thanks to Eric Dumazet for pointing out the fault in my first try to fix
this.

The problem is that the slave is first linked and slave_cnt is
incremented afterwards leading to a div by zero in the modes that use it
as a modulus. What happens is that in bond_start_xmit()
bond_has_slaves() is used to evaluate further transmission and it becomes
true after the slave is linked in, but when slave_cnt is used in the xmit
path it is still 0, so fetch it once and transmit based on that. Since
it is used only in round-robin and XOR modes, the fix is only for them.
Thanks to Eric Dumazet for pointing out the fault in my first try to fix
this.

Dave Chinner has properly pointed out that this is a deadlock in the
reclaim code because ext4 doesn't submit pages which are marked by
PG_writeback right away.

The heuristic was introduced by commit e62e384 ("memcg: prevent OOM
with too many dirty pages") and it was applied only when may_enter_fs
was specified.  The code has been changed by c3b94f4 ("memcg:
further prevent OOM with too many dirty pages") which has removed the
__GFP_FS restriction with a reasoning that we do not get into the fs
code.  But this is not sufficient apparently because the fs doesn't
necessarily submit pages marked PG_writeback for IO right away.

ext4_bio_write_page calls io_submit_add_bh but that doesn't necessarily
submit the bio.  Instead it tries to map more pages into the bio and
mpage_map_one_extent might trigger memcg charge which might end up
waiting on a page which is marked PG_writeback but hasn't been submitted
yet so we would end up waiting for something that never finishes.

Fix this issue by replacing __GFP_IO by may_enter_fs check (for case 2)
before we go to wait on the writeback.  The page fault path, which is
the only path that triggers memcg oom killer since 3.12, shouldn't
require GFP_NOFS and so we shouldn't reintroduce the premature OOM
killer issue which was originally addressed by the heuristic.

As per David Chinner the xfs is doing similar thing since 2.6.15 already
so ext4 is not the only affected filesystem.  Moreover he notes:

: For example: IO completion might require unwritten extent conversion
: which executes filesystem transactions and GFP_NOFS allocations. The
: writeback flag on the pages can not be cleared until unwritten
: extent conversion completes. Hence memory reclaim cannot wait on
: page writeback to complete in GFP_NOFS context because it is not
: safe to do so, memcg reclaim or otherwise.

Huge zero page is allocated if page fault w/o FAULT_FLAG_WRITE flag.
The get_user_pages_fast() which called in madvise_hwpoison() will get
huge zero page if the page is not allocated before.  Huge zero page is a
tranparent huge page, however, it is not an anonymous page.
memory_failure will split the huge zero page and trigger
BUG_ON(is_huge_zero_page(page));

After commit 98ed2b0 ("mm/memory-failure: give up error handling
for non-tail-refcounted thp"), memory_failure will not catch non anon
thp from madvise_hwpoison path and this bug occur.

Fix it by catching non anon thp in memory_failure in order to not split
huge zero page in madvise_hwpoison path.

Given a sufficiently large number of actions, while copying and
reserving memory for a new action of a new flow, if next_offset is
greater than MAX_ACTIONS_BUFSIZE, the function reserve_sfa_size() does
not return -EMSGSIZE as expected, but it allocates MAX_ACTIONS_BUFSIZE
bytes increasing actions_len by req_size. This can then lead to an OOB
write access, especially when further actions need to be copied.

Fix it by rearranging the flow action size check.

when get fiemap starting from MAX_LFS_FILESIZE, (maxbytes - *len) < start
will always true , then *len set zero. because of start offset is beyond
file size, for erofs filesystem it will always return iomap.length with
zero,iomap iterate will enter infinite loop. it is necessary cover this
corner case to avoid this situation.

After the first encap is attached, flow will be added to encap
entry's flows list. If neigh update is running at this time, the
following encaps of the flow can't hold the encap_tbl_lock and
sleep. If neigh update thread is waiting for that flow's init_done,
deadlock happens.

Fix it by holding lock outside of the for loop. If neigh update is
running, prevent encap flows from offloading. Since the lock is held
outside of the for loop, concurrent creation of encap entries is not
allowed. So remove unnecessary wait_for_completion call for res_ready.

After the first encap is attached, flow will be added to encap
entry's flows list. If neigh update is running at this time, the
following encaps of the flow can't hold the encap_tbl_lock and
sleep. If neigh update thread is waiting for that flow's init_done,
deadlock happens.

Fix it by holding lock outside of the for loop. If neigh update is
running, prevent encap flows from offloading. Since the lock is held
outside of the for loop, concurrent creation of encap entries is not
allowed. So remove unnecessary wait_for_completion call for res_ready.

The lock is used to synchronize different sysfs operations, it doesn't
protect any resource that will be touched by an interrupt. Consequently
it's not required to disable IRQs. Replace the spinlock with a mutex to fix
the deadlock.

The lock is used to synchronize different sysfs operations, it doesn't
protect any resource that will be touched by an interrupt. Consequently
it's not required to disable IRQs. Replace the spinlock with a mutex to fix
the deadlock.

Adding any sort of useful debug usually makes it go away, so I hand
wrote the function in a line, and debugged the asm.

Every so often pt->memory->ptrs is NULL. This ptrs ptr is set in
the nv50_instobj_acquire called from nvkm_kmap.

If Thread A and Thread B both get to nv50_instobj_acquire around
the same time, and Thread A hits the refcount_set line, and in
lockstep thread B succeeds at refcount_inc_not_zero, there is a
chance the ptrs value won't have been stored since refcount_set
is unordered. Force a memory barrier here, I picked smp_mb, since
we want it on all CPUs and it's write followed by a read.

v2: use paired smp_rmb/smp_wmb.

alloc_flex_gd(), it was possible for flex_gd->resize_bg to be bigger than
flexbg_size.

The reproduction of the problem requires the following:

 n_group: [o_group + flexbg_size, o_group + flexbg_size * 2)
 o_size = (n_group + 1) * group_size;

Take n=0,flexbg_size=16 as an example:

Delete the problematic plus 1 to fix the issue, and add a WARN_ON_ONCE()
to prevent the issue from happening again.

[ Note: another reproucer which this commit fixes is:

  img=test.img
  rm -f $img
  truncate -s 25MiB $img
  mkfs.ext4 -b 4096 -E nodiscard,lazy_itable_init=0,lazy_journal_init=0 $img
  truncate -s 3GiB $img
  dev=`losetup -f --show $img`
  mkdir -p /tmp/test
  mount $dev /tmp/test
  resize2fs $dev 3G
  umount $dev
  losetup -d $dev
