Why are you opening this PR?

This commit aims to improve the issue template to avoid opening new issues in
this project instead of the RERO ILS project. It also adds a triage label by
default to new issues, thus facilitating the management of new issues.

How to test?

Just read the commit changes.

Code review check list

  • Commit message template compliance.
  • Commit message without typos.
  • File names.
  • Functions names.
  • Functions docstrings.
  • Unnecessary commited files?
  • Extracted translations?

* Adds a warning on top of the template, as a reminder to open the issue
  in RERO ILS project, most of the time, except if it's a specific
  developer issue.
* Attributes by default the `triage` label, in order to identify easily
  new issues that are still not labelled or attached to a project or a
  milestone.
