This PR adds the current state of the rviz_plugin_covariance to rviz.

The use of rviz:: make the files close to the ones of the original plugin
While here, fix end of namespace comment
- Declare rviz objects explicitly in the header

- Add offset for orientation (used to position orientation shapes)

- Rename orientation_scale_node_ to orientation_root_node_

- Limit the angle to be displayed to 89 deg

- Add helper functions in anonymoous namespace
