open conifg

Deprecate open_frontend and add open configuration. It is
consistent with rdbg --open option.

RUBY_DEBUG_OPEN is also introduced.
For example,

$ RUBY_DEBUG_OPEN=true ruby -r debug script.rb

opens the debug port remotely.
