Display the full index on the manual #4194

For 0.4 I changed the stylesheet to only display one level in the TOC. I did this because, for the tutorial, I would much rather the reader see some text when they open the page than a big list of topics. This is less appropriate in the manual though and now it's difficult to find the material you want.

I'm not sure how to do this since we're not in control of the markup for the TOC. Maybe we can have multiple style sheets.
