Great! Mostly minor code suggestions. I think the main pattern we should try and align on is how to split our the create/update stuff. @mikewuu worked on this when we implemented the access code forms, and I feel like we mostly figured it out.
We don't have to solve that in this PR. If you can take a fist pass at some of the smaller things I flagged, we can come back to anything you want to move out of scope of this PR.
