You can't have two ids with the same value on the screen preferenceDialogForm. I would suggest prefixing/suffixing this with "what" it is

  • preferenceDialogForm-preferenceCenter
  • preferenceDialogForm-cancelDialog

or just naming them their own thing entirely...
