Table of Contents "toc-" prefix is visible in the URL hash

Current Behavior

We updated this to ensure unique IDs, but it's visible in the URL hash. We'll need to implement a quick adjustment to this. Even something like link- might be preferred, or a user-supplied param to replace this.

Another option: drop the prefix by default, opt for custom IDs on the headers to fix this. Maybe keep the param an optional setting.

I've opted to add optional prefix and suffix params and resolve the conflict on the Chat Blocks page manually via a custom heading ID.
