Ensure that most SDK calls don't result in hanging delegates (#12)

This tackles a handful of TODOs to call the failure case delegate
method, ensuring that _most of the time_ if one of the public APIs is
called, either of the appropriate delegate methods will also be called.

There's a couple others remaining that will also get addressed
separately.

Progress on #2 and #3.

Improve error consistency and details (#19)

This adds a number of new error cases, and ensures that we match up to
them when the AS delegate indicates an error.

Fixes #2 and makes progress on #3.
