Simplify Bank::clean_accounts() by removing params #27254

Problem

Calling Bank::clean_accounts() requires specifying multiple parameters that are alway the same for non-tests. This makes it possible to misuse inadvertently.

See #27200 for full context.

Summary of Changes

Set defaults for Bank::clean_accounts() and remove those from the fn params.

Since verify_snapshot_bank() happens at startup, I wanted to preserve calling AccountsDb::clean_accounts() with is_startup = true.

This call will change in the next PR too.

The test below, test_create_zero_lamport_with_clean is interesting. It probably isn't the right place to test that clean_accounts() is functioning correctly, but I didn't want to remove it as part of this PR.

So, Bank::clean_accounts(), now cleans up to but not including bank.slot(). This test relies on a specific slot getting cleaned so that the slot's zero lamport accounts are cleaned up. That means we need to make one more bank so that Bank::clean_accounts() will work on the intended slot.

Down in the test you'll see that the numbers are bumped by one; this is due to the extra bank I've added here.

this looks like the only one ever calling this with the 0th param = false.

Yes. I already changed all the other tests in Bank to use a test-only clean_accounts_for_tests().

I didn't know about this test in system_instruction_processor until today. I commented on it above: #27254 (comment)

lgtm if that test is the only one passing 'false' as param 0.

* reverts wide fanout in broadcast when the root node is down (#26359)

A change included in
#20480
was that when the root node in turbine broadcast tree is down, the
leader will broadcast the shred to all nodes in the first layer.
The intention was to mitigate the impact of dead nodes on shreds
propagation, because if the root node is down, then the entire cluster
will miss out the shred.
On the other hand, if x% of stake is down, this will cause 200*x% + 1
packets/shreds ratio at the broadcast stage which might contribute to
line-rate saturation and packet drop.
To avoid this bandwidth saturation issue, this commit reverts that logic
and always broadcasts shreds from the leader only to the root node.
As before we rely on erasure codes to recover shreds lost due to staked
nodes being offline.

#### Problem
get_entries_in_data_block() panics when there's inconsistency between
slot_meta and data_shred.

However, as we don't lock on reads, reading across multiple column families is
not atomic (especially for older slots) and thus does not guarantee consistency
as the background cleanup service could purge the slot in the middle.  Such
panic was reported in #26980 when the validator serves a high load of RPC calls.
