This PR closes a number of minor issues.

• remove the top level changelog fragment Stray changelog fragment in root directory #1576
• fix broken reference file table Reference file table formatting is broken and GAIN and READNOISE files are missing #1559
• fix elp documention description of arguments and fully saturated input handling Exposure pipeline documentation out-of-date #1524
• add saturation step to package index Saturation step missing from package index #1397
• remove unused items from resample step specification ResampleStep.single ignored #1388
