add files to contribute and code of conduct

Hello

This is a small contribution to this project to fill some gaps in the community perspective

Here we have:

1. code of conduct (important to keep the open source community out of troubles)
2. a contributors guide (with some basic info, we can enhance later)
3. a list of contributors (pretty basic so far)

I think will be nice have a CONTRIBUTORS file with this format: https://github.com/caolan/nodeunit/blob/master/CONTRIBUTORS.md

But I don't have time to do it properly
