[Bugfix][VLM] Fix failing Phi-4-MM multi-images tests and add vision-speech test

• Separate from [Model] Refactor Phi-4-multimodal to use merged processor and support V1 #15477 in case of potential regression issue.
• Fix Phi-4-mm divergence issues due to ineffective LoRA enablement. Because llm.llm_engine.add_lora(lora_request=lora_request) didn't enable LoRA during inference exactly.

Also cc @jeejeelee in case there is a bug at LoRA side.
