also, for a mouse that had to be re-stitched, where would this fall under ? they would have to be placed in the stereotax again and anaesthesia would need to be given

The form is to be complete when the wound treatment in first administered by the vets. It possible to add a end of treatment day to the form to account for the duration of the vets treatment.

As for re-stitching maybe an option for state whether the treatment is being redone may be the best course of action @gdevenyi ?

The idea is each form should be used for a "event in mouse's life"

If there is a begin/end then the form should have a begin mode, and an then be filled out again at the end.

If there's a "we need to resitch" the form should have a "we did a resitch" mode.

I haven't seen the recent form, so we should look at this together if its not clear.

Currently to form has a wound treatment section which is to be filled in with the form provided by the vet. I can add another re-stitching section which will include stephs suggestions.

Hi David, See my comments in my latest review of the form, I made suggestions for the restitching. I agree with Gabe here, the user should fill out the form to say they had to restitch the mouse. then if they needed additional treatment after the restitching, they can next fill out the wound treatment form. Second, for the end of treatment day, I don't think we can have this in the current form, since the vet doesn't usually predict how long the mice will receive the treatment. Maybe we should make a new form that enters an entry for when the treatment stopped. It would be a short form with treatment type, when it started and when it ended -- that's all you need since you'll already have the other info from when they first filled out the form that the mouse needed a treatment in the first place. Hi Steph,

I agree with the point being made, I'll adjust the form accordingly. As for the second point a wound treatment completion form can be created if needed.
