relation "mimiciii.ccs_dx" does not exist

When running to the file codes, the program reports an error

It's because MIT-LCP has chnaged their view creation code.
e.g., ccs_dx is not mimiciii schema anymore, but public schema

So, I made a pull request (#66) which simply changes search path of queries.
I hope it fixes your problem!
