Fully contained regions are deleted by repair. To keep the XML valid, they have to be removed from the ReadingOrder as well.

Remove deleted text regions from the reading order
47afd5f 
PAGE XML requires all elements referenced in the reading order
to be actually present as a region. With this commit, deleted
zones are removed from the reading order element which itself
is reindexed afterwards.
