xpad (xbox controller family) fixes #15

This fixes one crash, one kernel warning, one semi-broken feature and one obscure build failure.
More details on each individual commit message.

Tested as a dkms module on kernel 3.13.0(-29) in Ubuntu 14.04.

At startup with the pad already powered-on there's a small
window of time when we can start getting input packets
before there's a kernel input device set up to receive
events.  This would crash.

I know this happens with my xbox360 wireless pad (kernel 3.13.0)
and caused me occasional boot crashes.  The fix catches this.

I've put the same guard in the xpad and xboxone input routines;
I don't have appropriate devices but it seems like this can
only be benign.
move xpad_send_led_command outside of the #ifdef, since it's still
used either way.
Was re-using an urb without acquiring the lock.

Should fix kernel warnings of this nature:
"URB ffff88020d047d80 submitted while active"
Immediately after becoming present, we send the LED change
request to the pad.  There is then a race where the input
device may be opened and closed before the request's urb
is actually transmitted, in which case the LED change is
just dropped because of the usb_kill_urb().

This was actually happening almost all the time for me.

We really still want any outstanding output state changes to
get through, so just don't do usb_kill_urb() on the wireless
xbox360 device.

(This issue is perfectly likely to also affect other pad types,
but I've been conservative with the change.  It may also
help with a similar issue with dropped rumble state change
immediately before a close - unverified.)

Sadly adamdmoss@3e6434d doesn't fix the warning I was hoping it was fixing, but I believe the additional locking in that commit is still needed.

The reason for the oops is that IMA-appraisal uses "kernel_read()" when
file is closed. kernel_read() honors LSM security hook which calls
Apparmor handler, which uses current->nsproxy->mnt_ns. The 'guilty'
commit changed the order of cleanup code so that nsproxy->mnt_ns was
not already available for Apparmor.

Discussion about the issue with Al Viro and Eric W. Biederman suggested
that kernel_read() is too high-level for IMA. Another issue, except
security checking, that was identified is mandatory locking. kernel_read
honors it as well and it might prevent IMA from calculating necessary hash.
It was suggested to use simplified version of the function without security
and locking checks.

This patch introduces special version ima_kernel_read(), which skips security
and mandatory locking checking. It prevents the kernel oops to happen.

Dave Chinner has properly pointed out that this is a deadlock in the
reclaim code because ext4 doesn't submit pages which are marked by
PG_writeback right away.

The heuristic was introduced by commit e62e384 ("memcg: prevent OOM
with too many dirty pages") and it was applied only when may_enter_fs
was specified.  The code has been changed by c3b94f4 ("memcg:
further prevent OOM with too many dirty pages") which has removed the
__GFP_FS restriction with a reasoning that we do not get into the fs
code.  But this is not sufficient apparently because the fs doesn't
necessarily submit pages marked PG_writeback for IO right away.

ext4_bio_write_page calls io_submit_add_bh but that doesn't necessarily
submit the bio.  Instead it tries to map more pages into the bio and
mpage_map_one_extent might trigger memcg charge which might end up
waiting on a page which is marked PG_writeback but hasn't been submitted
yet so we would end up waiting for something that never finishes.

Fix this issue by replacing __GFP_IO by may_enter_fs check (for case 2)
before we go to wait on the writeback.  The page fault path, which is
the only path that triggers memcg oom killer since 3.12, shouldn't
require GFP_NOFS and so we shouldn't reintroduce the premature OOM
killer issue which was originally addressed by the heuristic.

As per David Chinner the xfs is doing similar thing since 2.6.15 already
so ext4 is not the only affected filesystem.  Moreover he notes:

: For example: IO completion might require unwritten extent conversion
: which executes filesystem transactions and GFP_NOFS allocations. The
: writeback flag on the pages can not be cleared until unwritten
: extent conversion completes. Hence memory reclaim cannot wait on
: page writeback to complete in GFP_NOFS context because it is not
: safe to do so, memcg reclaim or otherwise.

When a process runs out of stack the parisc kernel wrongly faults with SIGBUS
instead of the expected SIGSEGV signal.

This example shows how the kernel faults:
do_page_fault() command='a.out' type=15 address=0xfaac2000 in libc-2.24.so[f8308000+16c000]
trap #15: Data TLB miss fault, vm_start = 0xfa2c2000, vm_end = 0xfaac2000

The vma->vm_end value is the first address which does not belong to the vma, so
adjust the check to include vma->vm_end to the range for which to send the
SIGSEGV signal.

This patch unbreaks building the debian libsigsegv package.

James Morris reported kernel stack corruption bug [1] while
running the SELinux testsuite, and bisected to a recent
commit bffa72c ("net: sk_buff rbnode reorg")

We believe this commit is fine, but exposes an older bug.

SELinux code runs from tcp_filter() and might send an ICMP,
expecting IP options to be found in skb->cb[] using regular IPCB placement.

We need to defer TCP mangling of skb->cb[] after tcp_filter() calls.

This patch adds tcp_v4_fill_cb()/tcp_v4_restore_cb() in a very
similar way we added them for IPv6.
