GPU: Restrict mip CLUT enhancement a bit #15872

This makes the "second CLUT" from #15727 not trigger in most cases, except the safe case replicated by Misshitsu no Sacrifice. See #6357.

It also changes the GE frame dump so it'll include the extended CLUT data, and cleans up softgpu a bit so it won't try to load the extra data.

Can't replicate this behavior on a real PSP.

In case a game sets the separate CLUT flag by accident, ignore except
in a safe case that occurs in Misshitsu no Sacrifice.  See hrydgard#15727.
Not available on a real PSP, but used in an enhancement (see hrydgard#15727.)
We had 15 KB more space than needed before.
