Fix for: oxidecomputer/console#2700 (comment)
Also sneaking in a fix for hydration error with code blocks in the table of contents. Frustratingly if there's a code substitution it seems to automatically wrap it in a link. A little bit janky but here I'm removing the link if its there first – this is useful in other situations where people go rogue and add links to their titles.
