No stop button to interrupt agent

The AI keeps editing, but I cannot interrupt it without reloading the window:

Same is happening for me. I cannot stop it! The stop button shows but doesn't work.

No stop button -> all this work probably already happened, rendering is just behind for some reason

i haven't seen this - if you see this again can you check the output or console if there is anything there?

Still happens (and I cannot stop AI to edit my files, except by reloading VS Code):

I see one instance of this where an elicitation is dismissed (possibly because it ran a slow task) and then we don't reset the response state back to Pending.

@hediet please share a log for issues like this
