Add optional SAML conformance tests for Initiating Gateway and Responding Gateway actors. These run all the negative test cases (dealing with SAML assertion problems).

Add missing PV1 segment to A01 form of ADT message used in Patient Identity Feed message.

Fixed configuration issues where External Cache or Tomcat has space characters in path name. This occurs a lot on Windows systems.

Removed use of System.setProperty Java call. Causes errors on some Windows systems.
